REGISTRATION FOR THE 2012 SEASON

WHAT EVERYONE WILL NEED TO BRING (these items are mandatory!):

- Cash/Cheque Payment of $200 (taxes included) for Try outs

- Try out fee covers training camp tryouts, 2012 Spears Training Camp T-Shirt and Insurance

- POST DATED (May 1st 2012) CHEQUE for the remaining registration of $400 (taxes included)

- Registration fee covers- certified Spears equipment supplied for entire season, custom mouth guard, coach bus transportation to and from regular season away games, player polo, basic team picture, 1 banquet ticket for player and insurance.

- A parent/guardian if under the age of 18

- A photo copy of 2 pieces of ID (photo ID preferred) one being your health card
